Missing or expired proof load documentation

Proof load testing documentation is often the first thing an inspector asks to see. If a sling, spreader beam, lifting lug, hoist, or custom lifting device requires documented testing, verbal confirmation that it was tested is not enough. The record needs to identify the equipment, show the applicable test information, and be current for the job’s requirements.

Paperwork problems happen when certificates are stored in a different office, linked to the wrong asset number, lost during a crew change, or buried in an email chain. They also happen when a team assumes a previous test covers a new configuration, repair, modification, or project specification. It may not.

Before mobilizing, match every certificate to the exact equipment going to the site. Confirm serial numbers, identification marks, test dates, capacity information, and any customer or project requirements. If equipment was altered, repaired, or put back into service after damage, verify whether it needs retesting before use.

Equipment is damaged, worn, or visibly altered

Inspectors do not need a major failure to reject lifting equipment. A bent hook, stretched chain, cracked weld, damaged shackle pin, severe corrosion, illegible marking, or altered component can be enough to remove an item from service until it is evaluated.

The problem is often not that crews ignore obvious damage. It is that equipment slowly accumulates wear between formal inspections. Rigging gets dragged, exposed to weather, overloaded, struck by machinery, or used in ways it was not designed to handle. Paint, corrosion, abrasion, heat exposure, and normal handling can make tags and stamped markings unreadable. Homemade labels and marker-written capacities may not satisfy the documentation or identification requirements for the equipment. Check markings as part of your staging process. Equipment should have clear, permanent identification that ties back to its records. For custom fixtures and lifting beams, make sure the identification, rated capacity, and supporting proof load test documentation all agree. A clean, readable tag prevents an avoidable argument at the gate or in the field.

The equipment does not match the planned lift

Inspection failures frequently come down to application, not a broken component. A sling may be rated for a straight vertical lift but not for the angle planned. A shackle may not have adequate pin clearance. A lifting beam may be certified but not configured for the load, rigging arrangement, or center of gravity on the current job.

Capacity changes with sling angle, hitch type, basket configuration, connection method, and load distribution. The lowest-rated component in the system controls the lift. A certified piece of gear does not automatically make the complete rigging arrangement acceptable.

Review the pick plan before the equipment is sent out. Confirm load weight, center of gravity, attachment points, sling angles, environmental conditions, and clearance requirements. Verify that each component is compatible with the others, including pin size, hook throat, lug thickness, and hardware orientation. If the lift changes in the field, pause and reassess instead of forcing the original setup to work.

Required inspections were not performed or recorded

Many equipment categories require routine documented inspections in addition to periodic testing. A crew may perform informal daily checks, but if the required record is missing, incomplete, or inconsistent, an inspector may have no basis to accept the equipment for use.

This is especially common when equipment changes hands between shifts, projects, or subcontractors. One person assumes another completed the check. A form is filled out without asset identification. A defect is noted but never followed through to repair or removal from service.

Make the inspection record easy to use at the point of work. It should identify the asset, date, person completing the inspection, condition found, and action taken for any deficiency. The goal is proving that the gear was checked and that issues were controlled before the lift.

Unauthorized repairs or field modifications

Field repairs can create an immediate inspection issue, particularly on load-bearing equipment. Welding a lifting lug, replacing a component with non-equivalent hardware, drilling a new hole in a beam, straightening bent steel, or modifying a shackle arrangement can change the equipment’s capacity and invalidate prior assumptions about its condition.
